Care Choices HMO is a managed health care plan that has received an "Excellent" accreditation rating from the National Committee for Quality Assurance. Care Choices HMO offers employers a variety of benefit and pharmacy plans that include preventive health, hospitalization coverage and low copays. By partnering with our providers and hospital systems, we are able to provide the highest quality of care and service at an affordable cost. |

Care Choices PPO is designed to meet the needs of self-insured employers looking for a cost-effective, one-stop solution, with access to a national network of providers. Care Choices PPO is a superior integrator of care, connecting patients, providers and employers. Members receive the highest level of benefits in Michigan. For a higher out-of-pocket cost, members also have the freedom to use out-of-network providers across the U.S. |

Preferred Choices PPO provides access to a statewide network of 83 hospitals and more than 11,600 physicians. Care Choices HMO network includes 40 leading hospitals and more than 6,000 physicians. |
