Selecting a Preferred Choices Provider

Preferred Choices is a Network of Health Care Providers

Preferred Choices is a network of health care providers through which you can choose to receive your health services. The Preferred Choices Network consists of physicians, hospitals and other providers such as laboratories or home health care companies. When you choose Preferred Choices participating providers for your medical care, you can reduce your out-of-pocket costs based on the benefits of your medical plan.

Please Verify The Participation of Your Provider in Preferred Choices

It is your responsibility to check whether your provider is still participating. This directory is as complete as reasonably possible and is subject to change. Since some providers listed in this directory may no longer participate in the Preferred Choices Network, it is essential that you verify your provider’s participation prior to, or at the time of service.

You can do this by:

  • Inquiring directly with your provider whether or not they participate in Preferred Choices. When calling the provider’s office, simply ask if the provider "participates" with Preferred Choices. A provider must "participate" in Preferred Choices for you to have a reduction in your out-of-pocket expenses as defined by your benefit plan.
  • To obtain the lowest out-of-pocket expenses for services from another provider such as laboratory, radiology, or medical supplies please ask your physician to refer you to a participating Preferred Choices Network Provider.
  • Please note that if a provider participates with Preferred Choices at one office location, that does not necessarily mean that he/she participates at all of his/her office locations. Please also keep in mind that participation from one provider who practices in a group setting with other providers does not necessarily mean that all providers in that group participate in Preferred Choices.
