How to Obtain Health
Care Services
Follow this simple 5 step process to obtain services.
For further instructions, refer to additional information posted
further down on this page.
- Verify your eligibility and coverage before you receive services.
- Check the Preferred Choices PPO directory to select a participating
provider.
(Note: you can use our on-line directory or a printed copy)
- Verify that your provider participates with Preferred Choices
PPO.
- Present your PPO ID card at the provider's office (displaying
the Preferred Choices PPO logo).
- Verify that your provider's office will forward any claims to
the address listed on your PPO ID card.

Verify Your Eligibility And Coverage Before You Receive Services
It is important that you understand your health care benefits
and coverage levels are determined by your benefit plan. Preferred
Choices PPO is a provider network and not an insurance company.
Therefore, we are not involved with determining your eligibility,
benefits or coverage levels. Please check with your Benefit Plan
Administrator regarding eligibility, benefits, or coverage level
information; their phone number is located on your medical benefit
card.

How to Verify Your Provider Participates With Preferred Choices
PPO
The provider directory is as complete as reasonably possible at
the time of posting; however, provider information does occasionally
change. Since some providers listed in the directory may no longer
participate in the Preferred Choices PPO network or are practicing
at another location at the time you refer to the listing, it is
essential that you verify your provider's participation in the
Preferred Choices PPO network before you receive services.
Please note:
- It is important to ask if the provider: participates with Preferred
Choices PPO rather than ask if the provider accepts Preferred
Choices PPO.
- A provider may be willing to accept Preferred Choices
PPO which could mean that the provider is simply willing to care
for you even though the provider does not participate in Preferred
Choices PPO.
- Your Benefit Plan Administrator will consider the health care
claims from a non-participating provider as out-of-network and
you will not get a reduction in your out-of-pocket expenses.
- Your chosen health care provider must participate in
Preferred Choices PPO for you to have a reduction in your out-of-pocket
expenses as defined by your benefit plan.
Important Reminder: It is your responsibility to check whether
your physician, hospital or other health care provider is currently
participating in the Preferred Choices PPO network before you seek
health care services.
How To Obtain The Lowest Out-Of-Pocket Expenses For Services
From Providers
For services from another type of health care provider such as
a laboratory, radiology, or a medical supply company, please ask
your physician to refer you to a participating Preferred Choices
PPO network provider. These providers are listed in the Preferred
Choices provider directory.
Providers May Practice At Several Locations
It is important that you know that some health care providers
may practice at several locations and that the same providers may
not participate in the Preferred Choices PPO provider network at
all practice locations. In addition, not all physicians or other
health care providers within a group practice participate in the
Preferred Choices PPO provider network. Again, we encourage you
to verify that the specific physician or other health care provider
you choose participates with Preferred Choices PPO at your preferred
location before you receive services.
