Requirements for Participation in Preferred Choices PPO Network

Preferred Choices PPO physicians must comply with specific requirements to become or remain participating in the Preferred Choices PPO network. Physicians are required to:

• Have a valid license to practice as a physician in the State of Michigan
• Have a valid State of Michigan Pharmacy license
• Have professional liability insurance commensurate with specialty minimal coverage amounts of $100,000 per occurrence and $300,000 aggregate
• Be board certified or qualified to sit for exam, in the stated specialty of practice
• Have admitting privileges or arrangements at a contracted network hospital
• Meet our network need based on market needs or geographic access for members
• Provide full report of sanctions, and/or pending actions taken against practitioner’s license, including but not limited to the following: fraud, patient abuse, felony conviction, medical incompetence and professional misconduct
• Be absent of any sanctions for Medicare and Medicaid participation
• Be willing to cooperate and support the Preferred Choices PPO credentialing program
